TERMS OF SERVICE

By agreeing to our Terms of Service & Policies, you are making an offer to purchase goods which, if accepted by us, will result in a binding contract. Please note that products will not be sent until we have authorization from your payment card issuer. We will not be liable if there is a delay, and we will not accept your order if payment is not authorized.

After you have placed your order you will receive an email to acknowledge your order. It will confirm which products you have ordered, but it may not constitute an acceptance of your order.

We do not have to accept your order, and for example, we will not accept your order if:

  • We do not have the products in stock
  • Your payment is not authorized
  • There is an error on our website regarding the price or other details of the products
  • You have cancelled your order
  • You do not provide us with the proper Physician Order, if applicable, such as is required for CPAP Machines.

We reserve the right to refuse any order as well as cancel any subscription.

Please see individual product sales pages for information about delivery and shipping charges, where applicable.

For products that require shipping, we will email you as soon as your order has been shipped, and will advise of the shipping method (if any) at that time.

PAP MACHINE AGREEMENT

General:

  • After your completed order of a CPAP machine through www.cpapfast.com, we will send your sanitized machine with a power cord, carrying case, water chamber, standard tubing and filter. (Please note that the water chamber, filter and tubing are disposable items and need to be replaced on a regular basis).
  • Machines have a 90 day warranty. If they are broken, the Customer needs to send them back to CPAPFast and CPAPFast will replace the machine with a similar unit, which will have a new 90 day warranty against failure. Every attempt will be made to move the usage data from the broken machine to the replacement machine, but CPAPFast makes no guarantees about this. Additional length warranties may be purchased at the time of purchasing the Machine.
  • Machines under warranty, which break down and the Customer wishes to return for replacement, may be shipped to CPAPFast using the Customer’s chosen method of shipping. The Machine will be deemed received (The “drop off date” by CPAPFast once the unit arrives in our building. We highly recommend the Customer uses a shipping method that tracks the package to its end location. Alternatively, the Customer may purchase a return label by calling CPAPFast. In which case, the Machine will be deemed to have been returned (The “drop off date”) the day the package with return label is accepted by the carrier.
  • The warranty period begins the date the machine is shipped from CPAPFast’s facilities. The drop off date must be no more than 7 days (the grace period) past the end of the warranty period in order for the warranty to be valid.
  • CPAPFast also offers unlimited telephone support for any products sold by CPAPFast.

NOTICE: If I (the Customer) falsely dispute a charge to my credit card company there will be an additional $250 charge for increased administrative costs which I authorize to be charged to my credit card on file. This is in addition to the other costs outlined in the terms and conditions.

Authorization/Consent for Treatment:

I authorize CPAPFast under the direction of the prescribing physician, to provide home medical equipment, supplies and services as prescribed by my physician. I authorize CPAPFast to contact me via phone, text, mail and email regarding the services provided.

Returned Goods:

All sales are final. If there is a problem with a machine such that it is not functioning properly, CPAPFast will replace the machines, as long as it is inside the warranty period.

Damaged Equipment and Financial Responsibility:

In the event that the machine breaks down from normal use CPAPFast will replace my rental unit with another one at no addition charge, as long as it is still under warranty. I will contact CPAPFast to arrange sending in my broken unit to be replaced. Upon receiving the broken unit CPAPFast will confirm that the machine was not damaged due to negligence or purposeful damage. I, the customer, am responsible for getting the Machine back to CPAPFast. If I do not purchase a return label from CPAPFast, then I am responsible for tracking the package back to CPAPFast. If the machine is replaced by CPAPFast, CPAPFast will add an additional 90 days to the end of the current warranty period.

In the case of returning or trying to exchange broken or damaged equipment due to negligence or purposeful damage, the cost associated with repairing or replacing the device will be the sole responsibility of the Customer. In the event that I return broken/damaged equipment I hereby agree to pay CPAPFast for the balance to repair or replace the unit in full, within 30 days of receipt of invoice. All charges not paid within 30 days of billing date may be assessed as late charges. Late charges are $25 per week until the full amount is paid. I am also liable for all charges, including collection costs and all attorneys’ costs.

Authorization for Automatic Payment:

CPAPFast will require a credit card be kept on file to ensure that we are financially protected in the case a returned machine is broken and needs replaced or any addition fees occur.

I authorize The CPAP Box to execute transactions on the above account. I consent to the use of the above payment method without my signature on the individual transactions in satisfying my obligations to CPAPFast. I understand that a photocopy or fax of this agreement will serve as an original, and this payment authorization cannot be revoked unless done so in a 30 day written notice to the Provider.

  • I understand the charges.
  • If my credit card is declined, I will be charged an additional $25 service fee.
